The successful applicants will be based in one of the integrated community mental health Centre's in Ealing or Hounslow as part of the community and Recovery Mental Health Services service line. The service will comprise a fully integrated mental health team with a visible presence in the local Primary Care Network. There will be a tiered or stepped care model within the MINT team where a more primary care ethos and model of delivery will be used for the majority of patients but those who have more complex needs will have more enhanced support from an allocated clinician as well as the multidisciplinary team.
